Case Studies Session #1: Grounding Prevention

This presentation examines several grounding events discussing the casual factors and the impacts to health, the environment, and property. While each grounding had a different root cause, common behaviors that appeared in every event include complacency, distraction, inadequate planning, failure to follow rules and regs, and failure to identify hazards – all of which can be attributed to human behavior. Mike explains the proper procedure for avoiding assigning blame in each incident, rather showing how to identify system issues and opportunities for learning through proper incident investigation.
