AWO Near Miss 24-05 - Anchor Chain Quick Release Shackle Failure

In February of 2024, a third-party crew transport service was engaged to bring oncoming vessel crewmembers from their hotel to their assigned vessel.  While enroute, the captain noticed that the third-party van driver had their telephone propped on the dashboard of the van in front of the speedometer and was watching a television show while listening to audio through a Bluetooth headset.  The captain used Stop Work Authority and requested that the driver focus on the task at hand.
